If your dog has torn their cranial cruciate ligament (CCL), you may hear your veterinarian talk about something called a “pivot shift.” It sounds complicated, but it is actually an important clue about how unstable the knee has become.

The CCL in dogs is similar to the anterior cruciate ligament (ACL) in people. One of its jobs is to keep the shin bone (tibia) from sliding and rotating too much underneath the thigh bone (femur).

When the CCL tears, the knee becomes unstable. Many people have heard of the “drawer sign” or “cranial tibial thrust,” which are ways veterinarians test for front-to-back movement of the tibia. But there is another type of instability that can happen too: rotational instability.

This rotational instability is called a pivot shift.

What happens during a pivot shift?

When a dog puts weight on a knee with a torn CCL, the tibia rotates inward and slides forward. As the dog continues moving, the bones suddenly “shift” back into a more normal position.

This quick movement is called a pivot shift.

Why does it matter?

A pivot shift means the knee is not only unstable from front to back, but also unstable during twisting and turning movements.

This extra instability can:

  • Increase pain and discomfort
  • Damage cartilage inside the joint
  • Increase the risk of meniscal injury
  • Contribute to arthritis progression
  • Make athletic activities more difficult

Dogs that run, jump, play sports, or work in demanding jobs may be especially affected by rotational instability.

How do veterinarians detect it?

A veterinarian usually checks for a pivot shift while the dog is sedated or under anesthesia because the test can be difficult to perform in an awake dog.

During the examination, the veterinarian gently applies pressure and rotation to the knee while feeling for the characteristic “shift” of the bones.

Not every dog with a torn CCL will have an obvious pivot shift, but when it is present, it tells us important information about the severity of instability.

Can surgery fix a pivot shift?

Most common CCL surgeries, such as the Tibial Plateau Leveling Osteotomy (TPLO) and Tibial Tuberosity Advancement (TTA), are designed to control the abnormal forward movement of the tibia during weight-bearing.

These procedures are very successful at improving comfort and function in most dogs. However, some research has shown that a small amount of rotational instability may still remain after surgery in certain patients.

This is one reason why researchers continue to study knee biomechanics and ways to improve surgical outcomes.

What does this mean for my dog?

The good news is that most dogs do very well after CCL surgery and rehabilitation. Even if a pivot shift was present before surgery, many dogs return to walking, hiking, playing, and even sports activities.

A successful recovery depends on several factors:

  • Appropriate surgical treatment
  • Weight management
  • Rehabilitation exercises
  • Gradual return to activity
  • Long-term arthritis management

My take

The pivot shift is one of the reasons a torn CCL is more than just a simple ligament injury. The knee becomes unstable in multiple directions, which can affect cartilage, the meniscus, and long-term joint health.

While veterinarians often focus on drawer motion and tibial thrust, rotational instability is another important piece of the puzzle. Understanding the pivot shift helps explain why dogs with CCL injuries can develop arthritis and why surgery and rehabilitation are both important parts of recovery.

Most importantly, if your dog is limping or has been diagnosed with a CCL tear, early evaluation and treatment can help reduce instability and improve long-term comfort and function.
